Observed sanitizer at warewashing machine to be at 0ppm. Upon futher investigation found that the sanitzer tubing was placed in the dish detergne t and the tubing for the detergent was placed in the sanitzer. COS Assisted manager with placing the tubing into the correct chemical containers. Dish machine primed, cycled , and retested. Concentration at 50-100 ppm. Required CA: A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions P, and shall be used as follows:1. Observed hand washing sink in main kitchen to not be stocked with paper towels. Observed mens' bathroom to not be stocked with paper towels or any hand drying provisions. COS/CA: PIC restocked station with paper towels. RCA: Each handwashing sink or group of adjacent handwashing sinks shall be provided with:

Observed hand washing sink in main kitchen to not be stocked with any hand cleanser. COS/CA: PIC replaced soap dispenser. RCA: Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap.

Observed facility to not have any high heat test strips for dish machine. RCA: High heat test strips shall be obtained within 10 days.

Observed employee phone, speaker and charger to be stored amongst the dish storage area. COS/CA: PIC removed employee personal items and placed in designated area. RCA: Employee Items shall be kept separate from kitchen operations.
