8 - annual gas fired equipment inspection on file; gas units equipped with automatic safety and/or 100% cut-off pilot
Observed PIC not able to produce gas-fired equipment inspection report at time of inspection. Provided copy of blank report.
RCA: 511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed scuffing on all dressers inspected. Observed build-up of yellow substance in room 126. Observed cabinet in room 126 to be swollen from water damage.
RCA: PIC shall have company/LLC/owner or whomever is financially responsible to provide resources to restore furniture or replace furniture.
CA: 511-6-2-.14(5) - Condition of Furnishings. All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed staining on mattresses in all rooms inspected. See room list above. Observed black mold-like substance on mattress in room 218. Observed stains on mattress pads in rooms 326 and 335. Observed staining on pillows in rooms 126 and 232. Observed no mattress pad in room 126.
RCA: PIC shall have corporation/LLC/owner or whomever is financially responsible for repairs replace soiled mattresses or provide resources to treat the stains on mattresses as guests shall not sleep on soiled linens, soiled pillows, nor soiled mattresses.
CA: 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed toilet in room 207 was not cleaned nor sanitized. Observed clump of hair on shower wall in room 326.
RCA: PIC shall have staff clean bathrooms in rooms 207 and 326. CA: While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day
