3-1C - food in good condition, safe, and unadulterated
Regulation: 511-6-1.04(1) - safe, unadulterated and honestly presented (p)
Observed dry good cans such as 3 bean salad, baked beans, pinto beans, and chili Cuan corn with massive dents around the sides and top of it being isolated with "Dented Cans" sign and used in facility. COS: PIC and employees took the dry good cans which was stored in "Dented Cans" signs and discarded towards garbage.
6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed improper cold holding temperatures in reach-in cooler -Egg salad (54F), White American Cheese sliced (46F), Sliced Tomatoes (50F), -Ambient (61F).
COS: PIC moved items that were stored in reach-in cooler to walk-in cooler for proper cold holding within time/temperature control.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed reach-in cooler with door hanging off the hinges which causes it not to close properly for cold holding. Observed Reach-in Cooler with ambient of (61F) which is above cold-holding minimum temperature of (41F).
Observed Steamer leaking water which is turning into smokelike substance when the doors are opened.

- Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule.
- Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.05(3)(d) - ventilation hood system, adequacy, adequate to prevent grease & condensation build-up (c)
Observed Ventilation inadequate as evidenced by excessive steam, vapors in kitchen. COS: PIC called Maintenance to fix ventilation by turning back on breaker box/panel for specific equipment.
