Observed multiple food items (tomatoes, cheese, shrimp, burger, and ham) in prep top cooler holding above 41F. See temperatures in report. Reach in cooler seems to not be working properly, PIC stated that items were holding 41F and below this morning.
CA: PIC discarded all TCS foods that were out of temperature. Time/temperature control for safety food shall be maintained at 41°F (5°C) or below.

Observed black-like slime substance on the baffle of ice machine.
RCA: Equipment food-contact surfaces and utensils shall be clean to sight and touch.

Observed front panel of reach in cooler to not be in good repair and falling apart.
RCA: Equipment shall be designed and constructed to be durable and to retain their characteristic qualities under normal use conditions.

Observed raw burger patties stored above raw shrimp in reach-in cooler. Observed raw chicken stored directly next to and in contact with containers of cooked chicken.
CA: Employee rearranged items in reach-in cooler and walk-in cooler to follow separation guidelines.
Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food.
Except when combined as ingredients, separating types of raw animal foods from each other such as beef, fish, lamb, pork, and poultry during storage, preparation, holding, and display by using separate equipment for each type, or arranging each type of food in equipment so that cross contamination of one type with another is prevented, preparing each type of food at different times or in separate areas.

RCA: Educated PIC to clean dust and continue to monitor to prevent build up.
