Observed bike lock and chain on tennis court door to be loose and capable of entry via the door. This door is not self-latching/closing and shall remain closed and inaccessible at all times. Any openings in the gate or barrier located within eighteen inches of the release mechanism shall be less than one-half inch. 3. Mechanisms controlled by an access card reader, key entry device or keypad shall be located on the outside of the access gate.
Regulation: 511-3-5-.11(3)(g) - (g) the skimmer equalizer lines shall be protected by an approved cover/grated with a flow rating equal the maximum system flow divided by the number of skimmers in the system or the maximum flow rating of the skimmer, whichever is greater.

Observed visible silver mesh through decking near left side of pool deck at 5'3" marker. Observed patches of abrasive concrete that is peeling up upon contact. These patches were observed on the right side of the pool near the shallow end. RCA: Pool deck shall be smooth, continuous and well maintained. Patches are permitted if they do not create a lip or tripping hazard. Pool deck shall be resurfaced within timeline provided in the Plan of Action.

Observed toilet in women's restroom not operable. Observed chairs in women's restroom blocking the area. Observed no paper towels or soap in men's or women's restroom. RCA: Restrooms shall remain in good repair and be stocked appropriately.
Observed light pulling away from internal side of pool creating a gap between pool wall equipment piece. RCA: Pool walls shall be smooth and free from any possible entrapments.
Observed missing tile revealing foam and internal structure of pool within right side skimmer wall at 5'3" marker. Observed tiles at skimmer wall on right side of pool at 5'3" marker to be pulled away and not secured by grout. Observed cracked tile with sharp edge on right side of pool at 5'3" marker. RCA: Pool shall be seal, water tight and free from sharp protusions.
