4-1A - food separated and protected
Regulation: 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - packaged & unpackaged food separation, packaging, and segregation (p, c)
Observed improper separation of raw animal products in the reach in cooler in the back of house near the 3-compartment sink. Raw chicken was stored above raw whole cut pork and beef. CA: PIC placed chicken at bottom of cooler.
4-2A - food stored covered
Regulation: 511-6-1.04(4)(c)1(iv) - packaged & unpackaged food, food stored covered(c)
observed wontons, crab rangoon, and egg rolls all stored uncovered, not in active use and not protected from overhead contamination in the reach in cooler underneath the grill. Observed raw chicken no covered in the reach-in cooler in the back of house near 3-comp sink. Observed every food item in prep top cooler (Shrimp, Beef, Carrot, Chives, sauces) to not have coverings and not in active use. CA: PIC covered all items not in active use. RCA: storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
Observed dishwashing machine in active use and not producing a chemical residual after sanitation cycle. Chemical being used is chlorine and it is not testing. CA: PIC contacted repair company to service the unit and will discontinue using the machine and wash dishes in 3-comp sink. RCA: Dishwasher should be capable of producing a chlorine residue of 50-100ppm on dish surface following dish cycle.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
observed sushi, preptop, and back of house reach in coolers in the facility to have food debris build up on the front of the equipment, as well as, within the gaskets. CA: PIC shall deep clean all equipment non-food contact surfaces. RCA. RCA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed food debris splatter on walls and ceilings throughout the main kitchen and the back of house area near 3-compartment sink, and dry storage area. CA: PIC shall deep clean the facility. RCA: The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
