Observed milk in walk in cooler at 49F. Maintain all TCS foods at 41F and below.

Missing tiles in threshold of walk in cooler, Tiles in back are chipped/damaged - no longer smooth easily cleanable, Paint/walls above serving windows deteriorating and chipping.
Make repairs to facility so that floors, walls, ceiling are smooth and easily cleanable.
