Observed small gap under back door. Outer openings must be protected from pest entry. Must fix door sweep or adjust door to be tight fitting.

Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed several cracked tiles in main kitchen near the 3 compartment sink and back door. Also observed cracked tile cove basing next to mop sink, and gaps around utility lines above water heater. All physical facilities must be maintained in a state of good repair. Must fix

Observed unfinished wooden panel behind water heater near 3 compartment sink. Floors, walls, and ceiling must be designed so they are smooth and easily cleanable. Must seal or replace.
