Observed kale brought out of temperature control around 10 AM, without proper time documentation until 10:25 AM. Must have time documentation on or near product with the time the food was brought out of temperature control, and the discard time 4 hours later. Must have documentation as soon as product is brought out of temperature control. COS by adding TPHC label.
