Regulation: 511-6-1.05(7)(b) - food contact surfaces and utensils - cleaning frequency (p, c)
Observed mold build-up along interior portions of bulk ice machine in main kitchen. Ice machine must be emptied, cleaned, and sanitized within 72 hours. Must increase cleaning frequency to a rate that precludes mold accumulations.

Observed ice scoop stored on a visibly dirty (molded) surface. Equipment food-contact surfaces and utensils shall be clean to sight and touch. COS by washing both.

Observed dish machine dispensing chlorine at 10 ppm. Must dispense at 50 ppm. COS by priming dish machine. Chlorine then dispensed at 50 ppm.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Ceiling tiles in pizza area are in poor condition--visibly dirty and not in place. All physical facilities shall be maintained in good repair. Ceiling tiles more than likely need to be replaced.
