Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
Person in charge (PIC) not able to demonstrate in a verifiable mannerthat food employees are informed of their responsibility to report symptoms and illnesses. Corrective action (CA): All workers shall read & sign the employee reporting agreement, & signed agreements shall be kept on-site for verification by Health Dept.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed cracked/damaged floor tiles in dry storage area.
