Observed mold-like substance in bulk ice machine. Corrective action (CA): Clean ice machine frequently enough to preclude growth.

Observed multiple food workers prepping food without hair restraints. Food workers all put on hair nets.
17C - physical facilities installed, maintained, and clean

Mops stored on floor next to service sink; not hung to dry. Person in charge hung mops.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(5)(j) - using dressing rooms & lockers, used for changing clothes and storing employee belongings (c)
Employee drinks stored on top shelf under make table. Person in charge moved drinks to proper location.
