Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
PIC could not produce signed employee health reporting agreements. Advised must have all employees sign and documentation kept at facility. CA: Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food.
11A - proper cooling methods used: adequate equipment for temperature control

Observed cooked steak that had just been prepped and placed in the walk in cooler tightly covered while in the cooling process. COS: PIC loosely covered. CA: Use proper cooling methods to rapidly cool TCS food items.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(a) - good repair & proper adjustment (c)
Observed no closing lids on front service prep top cooler. Advised to repair. CA: All equipment shall be kept in good repair and proper adjustment.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed bottle of cold brew coffee stored in walk in freezer with facility items. COS: PIC moved to designated area. CA: Use designated areas for employee personal items to prevent cross contamination.
