Observed missing smoke detectors in the following rooms: 114, 108, 124. Advised PIC all furniture, draperies, appliances, carpets, and other accessories and must be maintained in good repair

Observed stains on linens in the following rooms: 114 (Sheets/Pillows), 226 (Sheets), 124 (Pillows). Advised PIC beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean

Observed in room 114 that a black substance was on the ceiling of bathroom. Observed buildup on the vent within bathroom. Advised PIC that the current ventilation must prevent odor, mold, mildew, and moisture

Observed chairs in rooms 216, 224, 211, 121 in need of repair or replacement. Also observed missing smoke detectors in rooms 216, 22, 121, 11, & 211. Advised to put back up. CA: Furnishings shall be maintained in good condition and repair

Observed vents in bathrooms in rooms 100, 121, 211, 216, & 224 with build up and in need of cleaning. Advised to clean. CA: Ventilation shall be properly maintained

Observed unlabeled bottles of cleanser in laundry room. Advised to label. CA: Cleansers outside of original container shall be labeled with common name of product

Observed smoke detectors in rooms 121, 200, 206, and 226 in need of being put back in place. Also observed chair in need of replacement in room 115. Observed microwave in room 121 with heavy rust and in need of replacement. Advised to replace chair and microwave and ensure smoke detectors are put back into place and working

Observed crumbs under chair cushions in rooms 114, 104, 100. Advised to clean under cushions. Also observed build up in microwave in room 104 and microwave in need of replacement in room 100. Advised to clean/replace. CA: All furnishings shall be kept clean and in good repair

Observed ventilation not working in bathroom of room 114. Advised to repair. CA: Bathrooms shall have proper ventilation

Observed no anti-slip surface on tubs in rooms 114 & 227. Advised to install anti-slip applique. CA: Tubs and showers shall have slip resistant surfaces

Observed chairs in need of replacement in many rooms. Observed crumbs under cushion on chair in room 114. CA: Furnishings shall be kept clean and in good repair

Observed no anti-slip surface on tubs in rooms 110, 114, 116, 216, 224, & 228. Advised to install anti-slip applique. CA: Tubs and showers shall have slip resistant surfaces

Observed no current annual gas fired equipment inspection form on file at facility. Last gas fired equipment inspection expired in August 2023. Advised PIC it must be performed annually. PIC stated he had appointment to have inspection done

Observed chairs with heavy stains in rooms 102, 104, 120, 122, 210, & 232. Also observed crumbs under cushions of chairs in all the above rooms. Advised PIC to clean. Also observed broken vent covers on a/c's in rooms 104, 122, & 232. Advised PIC to repair. Observed gaskets on fridges in rooms 104 & 210 in need of repair

Observed cigarette burns on tubs in rooms 102, 104, 232

Observed heavy dust build up on vents in rooms 104120, 122, 210, & 232. Advised PIC to clean
