Observed food contact surface of the interior of the bulk ice machine stored as clean with severe black mold-like substance. Advised PIC that equipment food contact surface and utensils shall be cleaned at any time during the operation when contamination may have occurred. Advised PIC to increase the cleaning frequency of the bulk ice machine.
11A - proper cooling methods used: adequate equipment for temperature control

Observed a bin of cooked rice stored covered cooling at room temperature on the counter in the main kitchen. Advised PIC on proper cooling methods: under refrigeration covered/tightly covered. COS: PIC placed bin of cooked rice in the walk-in cooler for proper cooling.
18 - insects, rodents, and animals not present
Regulation: 511-6-1.07(2)(k) - insect control devices (c)
Observed an insect control device stored directly above single-service cup lids. Advised PIC insect control devices shall be installed so that the devices are not located over a food preparation area, and dead insects and insect fragments are prevented from contact with exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles. Advised PIC to relocate insect control device.
