11A - proper cooling methods used: adequate equipment for temperature control
Regulation: 511-6-1.04(6)(e) - cooling methods (pf, c)
Observed a container of shredded cheese cooling in a disrepaired reach in cooler in the front-service area. Advised PIC Cooling shall be accomplished in accordance with the time and temperature criteria specified under DPH Rule 511-6-1-.04(6)(d) by using one or more of the following methods depending on the type of food being cooled using rapid cooling equipment. COS: PIC placed shredded cheese in the walk-in cooler for proper cooling.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(f)(1) - permit, displayed (c)
Upon arrival, observed the 2025 Food Service Establishment Permit not posted in public view. Advised PIC that a current Food Service Establishment Permit shall be posted in public view at all times. Advised PIC to relocate the 2025Food Service Establishment permit so that it is in public view.
15A - food and nonfood-contact surfaces cleanable, properly designed, constructed, and used
Regulation: 511-6-1.05(6)(q)1&3 - good repair & calibration (c)
Observed the front-service reach in cooler with an ambient temperature of 43.5F. Advised PIC ambient air temperature, water pressure, and water temperature measuring devices shall be maintained in good repair and be accurate within the intended range of use. Advised PIC to have unit serviced.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed nonfood contact surfaces of the interior of the front food service reach in cooler and the interior of the walk-in freezer with food debris and residue. Advised PIC nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. Advised PIC to thoroughly clean nonfood contact surfaces of the front food service reach in cooler and the walk-in freezer.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed ceiling tiles near the front food service area in need of cleaning due to staining. Advised PIC the physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. Advised PIC to thoroughly clean ceiling tiles in the front food service area.
