Advised PIC all gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions

Observed no inspection report posted in public display at time of inspection. Advised PIC the most recent tourist accommodation inspection report form shall be prominently displayed in public view at all times, between five feet and seven feet from the floor and in a public area such as the registration desk, where it can be read at a distance of one foot away

Observed no permit displayed at time of inspection. Advised PIC any person operating a tourist accommodation shall obtain and display a valid DPH tourist accommodation permit issued by the Health Authority

Observed interior food contact surface of bulk ice machines (2nd, 4th, and 6th Floor) with stains, debris, and pink-mold like substance. Advised PIC In existing tourist accommodations permitted prior to the adoption of this rule, the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

Employee moved chemicals from single-service and single-use articles

Observed staining and debris on mattress in rooms 700, 408, 403, 210. Advised PIC that all mattresses, box springs, and pillows shall be maintained and kept clean at all times. Advised PIC to thoroughly clean/replace mattresses

Observed guestroom water temperatures exceeding 120 degrees F. in rooms 721 (123F), 619 (121F), 432 (123F), 121 (125F), 617 (121F), 718 (124), 618 (125F). Advised PIC that all guestroom water temperatures shall not exceed 120 degrees F
