Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed multiple utensils (2) stored as clean with heavy accumulation and food stains. Advised PIC equipment food-contact surfaces and utensils shall be clean to sight and touch. COS: PIC relocated to 3-compartment sink for wash-rinse-sanitize.

Regulation: 511-6-1.02(1)(f)(1) - permit, displayed (c)
Observed 2025 permit posted at time of inspection. Advised PIC to post the permit in a location in the food service establishment that is conspicuous to consumers.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed rusting on shelving in WIF on non-food contact surface. Advised PIC nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed multiple stains on ceiling tiles of facility in multiple areas. Advised PIC the physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(3)(h) - dressing areas and lockers, designated and provided (c)
Observed personal belonging, coats stored on top of soda bottles. Advised PIC Lockers or other suitable facilities shall be provided and used for the orderly storage of employees' clothing and other possessions. COS: Had PIC relocate personal belongings to designated area.
