Observed bulk milk poured into separate pitcher for guest use. To comply with continental breakfast rules, milk must be individual use and pre-packaged from manufacturer.
C/A: A tourist accommodation without a food service permit may serve a continental breakfast as defined in DPH Rule 511-6-2-.03(d); foods not requiring preparation and cooking; potentially hazardous foods commercially prepared and packaged in single-servings; non-potentially hazardous foods commercially prepared and packaged and served from the original container or an approved dispenser such as bagels or bread that may require only reheating or toasting by tourists, if the conditions of subsections 2(a) through (j) are met

Observed frozen waffles that were being stored at room temperature for guest use. Frozen items must be cooked from frozen and not stored at room temperature.

Observed accumulation of dust and dirt on the floors in all rooms, especially along the edges and in corners.
C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean
15 - ice from approved source; properly handled, supplied and stored; equipment approved/clean

Observed black debris stains on ice machine.
CA: Clean the ice machine regularly to prevent buildup
