Observed in rooms 526, 501, 423, 312, 201, and 232, mattresses with large yellow stains and no mattress cover/bedbug mattress cover. Observed in room 501, large red/brown stain on side of mattress and on box spring cover. Observed in room 423, red stain on mattress box spring. Observed in room 312, a loose screw between box spring and mattress, stain on box spring, and pillow with yellow stain. C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed ice machine on 3rd floor with a buildup of pink and black slime mold. C/A: The machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

Observed no water and fire gas inspection on premises at the time of the inspection. CA: Evidence of a completed gas-fire inspection should be provided at time of health inspection

Observed staining on towels, mattress, pillows, pillowcases, and fitted sheets in rooms 329, 523, 531, 504, and 502. CA: Beds, beddings, and linens should be clean and free from stains

Observed loose drain stopper in room 329. Observed loose tub faucet in room 504. CA: Shower and tub facilities should be tightened, clean, and in good repair

Observed residue on bottom of ice machines on 3rd, 4th, and 5th floors.

Ice machines should be properly cleaned and sanitized

Observed no water and fire gas inspection on premises at the time of the inspection CA 511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency FACILITY HAS 30 DAYS TO PROVIDE A COPY OF THE GAS-FIRED INSPECTION REPORT UPDATE: Facility sent a PDF file with the inspections done. They do have the inspection date as 12/31/2025 instead of 12/31/2024. 12/31/2025 is the expiration date

Observed the sofa bed mattress in room 532 ripped on the side and the coils were showing. Observed stains on mattress cover in room 421. Observed stains on pillows in room 421 and 311 Observed stains on mattress in room 122, 311 and 401 C/A 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly
