Observed stains on the mattresses in rooms 311, 403, and 402.

Observed room 411 had stains on the mattress and room 419 had stains on the mattress pad. Observed no mattress cover or protector on any of the beds.

I observed the water the water exceeding 120 degrees in the following rooms 421, 420, 415, 410, 310, 315, 320Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F. C/A

I observed stains on pillows in the following rooms nr 421, 420, 320 I observed a few stains on mattress in the following rooms 421, 415, 410, 315 I observed stained mattress covers in the following rooms in the following rooms 320, 410 Need to change the mattress pad in the room 410 I observed some holes and very worn on the underside. 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly.
