Observed water temperature exceeding 120F in all rooms inspected.

C/A: hot water in all guest rooms shall not exceed 120F

Observed table with sticky residue in room 228. Observed dresser drawers with loose debris in room 133. Observed a ripped and stained chair in room 309.

C/A: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

Observed trash under the bed in room 133. Observed stains/residue on bathroom floors in room 220. Observed soiled carpet with white substance in room 228. Observed dusty floors under the bed in room 309. Observed hallway floors with debris, stains, and rips. Observed elevator with q-tip and chicken bone on the floor.

C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed water exceeding over 120F in the following rooms: 128 = 125F, 228 = 124F, 313 = 125F, 314 = 122F, and 328 = 122F.

CA: In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F. Facility turned down hot water boilers to 120F during visit

Facility has no documentation that the annual gas fired equipment inspection has been conducted during the past 12 months. CA: ll gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed stains on pillowcases and sheets as well as debris on the mattress covers.

CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

Observed toilet in room 128 that seemed to have been recently used with stains. Observed room 228 with stains on the toilet seat.

CA: Toilets must be cleaned and sanitized in between guests or at least once a week

Observed stained mattress covers in room 101, 328, and 211. CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

I observe the hot water exceeding 120degrees in the following rooms 321, 131, 106, 109, 108 511-6-2-.07(2) - Cold Water Supply Cold running water under pressure shall be provided to all equipment that uses water. Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 1200F.

I observed stained mattereses in the following rooms 32`, 108 I observed dirty office chairs in the following rooms 321, 106 I observed stains and food remnants on the bed skirt in room 321 CA 511-6-2-.14(6) - Bed, Bedding, and Linens Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

I observed the coating in the shower tub being worn out in the following rooms 321, 131, 106, and 108. Need a slip matt or replace. CA 511-6-2-.08(10) - Anti-Slip Surface Anti-slip tubs slip strips, appliqués, or slip-proof mats shall be provided in each bathing facility and shall be kept clean and in good repair

Mattress pads were missing on the sleeper sofa in room 114 and bed mattress in room 109. CA:Washable mattress pads or covers shall be used on all mattresses

Hot water temperatures exceeded 120F in the following rooms - 114 (132F), 102(128F), 139(133F), 215(136F), 317(126) 321(130F). CA: Hot water in all guest rooms shall not exceed 120F

PIC was not able to produce Annual Fire Inspection report. CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority

Observed stains on mattresses in rooms 114, 215, 317. Observed stain on mattress cover in rooms 321. There was no mattress covers in room 114 and 317. CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin
