C/A: Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed

Observed live roaches in room 240.

C/A: Effective and appropriate measures shall be taken to eliminate the presence of rodents and flies, roaches, bed bugs, and other insects on the premises. The premises shall be kept in such condition as to prevent the attraction, harborage or feeding of insects or rodents

Vents were not working in the bathrooms of all rooms inspected.

C/A: Toilet, lavatory and bathing facilities shall be mechanically ventilated

Window screens were not present on the windows in rooms 233, 240, 343, and the hallway windows of the second and thirds floor.

C/A: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means

Facility unable to provide annual gas-fired equipment checklist for inspection. C/A: Checklist must be current and available at time of inspection

Observed stained mattress in rooms 105 and 331. Observed holes in sheets in rooms 124 and 343. Observed a hair in sheets in room 343. C/A: Bed, bedding, and linens shall be clean, in good repair, and free from stains

Observed windows with missing screens in rooms 105, 124, and 331

Observed missing ceiling tiles in hallway from active water leak. C/A: Floors, walls, ceilings shall be in good repair and sound construction

Observed ice machine with build-up on the 1st floor. C/A: Ice machines shall be cleaned, sanitized, and in good condition

Observed stained mattresses in rooms 125, 220, 214, and 203. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin

observed gas fire inspection from 05/04/2023 which was expired CA 511-6-2-.15(5) - Heating and Fire Safety-Inspection All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed rooms 341 and 345 and the ventilation was not working CA 511-6-2-.08(5) - Toilet and Bathing Ventilation Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture. All new or major renovation of a tourist accommodation will require mechanical ventilation to be installed to the outside of the building separate from heating and air-condition systems

Observed black residue on ice machine dispenser. CA: Ice storage and dispensing equipment shall be maintained in clean and sanitary condition. Clean and sanitize ice dispenser within 48 hours. Verified ice machine dispenser was cleaned during informal inspection conducted 1/08/24
