4-1A - food separated and protected

Raw beef was stored above a container of fries inside the refrigerator. CA: Food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables. COS: The raw beef was stored on the bottom shelf of the refrigerator.

Containers of vegetables and wonton chips stored inside the reach in cooler, walk in cooler and on the prep table was stored uncovered. CA: Storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. COS: The food items were covered.
12A - contamination prevented during food preparation, storage, display
Regulation: 511-6-1.04(4)(q) - food storage (c)
Containers of soy sauces, bottle water, flour, and vegetable oil was stored on the floor inside the kitchen. CA: Food shall be protected from contamination by storing the food in a clean, dry location where it is not exposed to splash, dust, or other contamination at least 6 inches (15 cm) above the floor.
12C - wiping cloths: properly used and stored
Observed wet rags on countertops in main kitchen area. CA: Cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be maintained dry; and used for no other purpose. Cloths in-use for wiping counters and other equipment surfaces shall be held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n). COS: The wiping cloths were removed.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed oil, debris and accumulation on hood vent and wall behind cooking section. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
17C - physical facilities installed, maintained, and clean

Observed backdoor in disrepair and screen coming loose. CA: All physical facilities shall be maintained in good repair.

Food debris, grease and dirt was observed on the floor underneath the fryer and stove. CA: All physical facilities shall be maintained in good repair.
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(3)(f) - lighting intensity, adequate in food prep, storage & service areas (c)
A light was not available inside the dry storage area near the walk in freezer. CA: The light intensity shall be at least 10 foot candles (108 lux) at a distance of 30 inches (75 cm) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning.
