Regulation: 511-6-1.03(5)(c) - when to wash (p)
Observed employee prepping raw chicken, touched boxes on floor to throw away outside, returned prepping raw chicken without washing hands or new gloves. C/A-Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles P and:
(i) After touching bare human body parts other than clean hands and clean, exposed arms; P
(ii) After using the toilet room; P
(iii) After caring for or handling service animals or aquatic animals; P
(iv) After coughing, sneezing, using a handkerchief or disposable tissue, using tobacco, eating, or drinking, except for drinking from a closed beverage container and the container is handled to prevent contamination of the hands; P
(v) After handling soiled equipment or utensils; P
(vi) During food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks; P
(vii) When switching between working with raw food and working with ready-to-eat food; P
(viii) Before donning gloves to initiate a task that involves working with food; P and
(ix) After engaging in other activities that contaminate the hands. COS- EHS asked employee to wash hands and put new gloves on.

Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. COS- Food prep employees put on hair restraints
Observed FS sanitizer bucket at 100ppm QAC. Observed several wiping cloths in main kitchen sitting out food prep top tables. C/A-Wiping cloths shall be held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n) COS- PIC remade sanitizer bucket to 200ppm QAC and wiping cloths placed in sanitizer bucket

Observed two large containers and two small containers of egg salad in FS display cooler that were cooling and tightly covered with lid. C/A-L\When placed in cooling or cold holding equipment, food containers in which food is being cooled shall be loosely covered, or uncovered if protected from overhead contamination during the cooling period to facilitate heat transfer from the surface of the food. COS- PIC pulled items from front service and placed them in freezer uncovered for rapid cooling

Observed several food boxes on floor of WIC. PIC stated a truck was delivered this morning; they are short staffed and have not been able to move food off floor yet. C/A- Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; (iii) At least 6 inches (15 cm) above the floor.

C/A- Wiping cloths shall be held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n) COS- PIC stated they are currently out of sanitizer but do have sanitizer wipes that tested at 400ppm QAC. PIC dumped sanitizer bucket and replaced with a pack of sanitizer wipes
