Observed no posted SERV safe food manager certificate for food manager; EHS observed certification at last inspection.
C/A: The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority.
Post food manager certificate within 10-days and send photo to EHS.
4-2B - food-contact surfaces: cleaned & sanitized

Observed ice machine in back storage area of main kitchen with pink and black accumulations along interior and exterior of unit. Ice machine also located directly under hole in ceiling that creates potential contamination from outside.
C/A: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
COS: Ice machine relocated and cleaned while on-site.
6-2 - proper date marking and disposition

Observed sour cream held beyond manufacture's best by date (7/2024) in RIC in main kitchen.
C/A: A food that requires datemarking shall be discarded if it:
(i) Exceeds 7 days, not including the time that the product is frozen;
(ii) Is in a container or package that does not bear a date or day; P or
(iii) Is appropriately marked with a date or day that exceeds 7 days .
COS: Food discarded.
6-2 - proper date marking and disposition

Observed sliced American cheese held for more than 24 hours not date marked in RIC in main kitchen.
C/A: refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days.

Observed no employee hand-wash signs at hand-wash stations in restrooms or throughout kitchen/bar.
C/A: Handwashing Signage. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees.
EHS provided signs to PIC.
13A - posted: permit/inspection/choking poster/handwashing
Regulation: 511-6-1.02(1)(d) - displaying of the inspection report (c)
Observed no posted CDPH inspection in public view within facility.
C/A: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
14B - utensils, equipment and linens: properly stored, dried, handled
Regulation: 511-6-1.05(10)(e)1,2,4 - equipment, utensil, linens, stored 6" off floor in clean, dry location (c)
Observed several boxes of single use articles stored directly on the floor of storge room in main kitchen.
C/A: Cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored:
(i) In a clean, dry location;
(ii) Where they are not exposed to splash, dust, or other contamination; and
(iii) At least 6 inches (15 centimeters) above the floor.
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(d) - nonfood-contact surfaces (c)
Observed fan grates in WIC in main kitchen heavily soiled with dirt and organic matter.
C/A: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
- Observed missing floor tiles in WIC in main kitchen.
- Observed ceiling in disrepair; hole in ceiling above ice machine in back storage area of facility.
C/A: Repair all areas; all physical facilities shall be maintained in good repair.
Owner must provide photo to EHS of ceiling repair within 3-days.
