Observed pickled onions stored uncovered in reach in cooler 3. C/A: Food shall be stored covered in order to prevent contamination. COS: Items covered

Observed dish machine dispensing 0 ppm chlorine with dishes inside. C/A: Chlorine sanitizer should be maintained between 50-100 ppm. COS: PIC primed machine; chlorine measured 100 ppm.

Observed clorox wipes and "project plan" chemical spray bottle stored directly next to plastic utensils and above salt and pepper shakers in the banquet prep room. C/A: Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles by:
- Separating the poisonous or toxic materials by spacing or partitioning. COS: Items moved to designated chemical storage.

Observed employees phone stored on a storage bin containing pasta noodles. C/A: Employees must store their personal belongings in a designated area only.
