Observed food employee throw trash away outside then returned to kitchen and start prepping chicken without washing hands first. C/A: Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation. COS: Hands washed.

Observed several TCS foods cold holding above 41F on counter tops (main kitchen). Observed large bowl of sweet and sour soup cold holding above 41F in reach-in cooler. Observed shredded and cooked cabbage sitting in sink cold holding above 41F C/A: Cold holding TCS foods shall be 41F or below at all times. COS: PIC placed items back into cooler. PIC stated items were taken out of cooler for half an hour.

Observed chicken and veggies hot holding below 135 on grill. C/A: TCS hot holding foods shall be 135 for above at all times. COS: PIC turned grill on to rapidly reheat the foods.

Observed large containers of sliced chicken and pork not cooling rapidly. C/A: TCS foods being cooled from 135F to 70F in 2 hours must cool 32.5F per hour. COS: PIC instructed food employee to place chicken in smaller portions and on ice.
