Observed floors with dust and debris accumulated, especially around the edges of the rooms. Observed dead bugs in rooms 124 and 116. Observed broken/separating baseboards in room 117.

C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Rooms 125, 124 and 117 were observed to have few stains and loose hair on bedding.

CA: Beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings should remain clean.

COS: PIC removed bedding for cleaning

Observed missing pest control records on file for several months in 2023 and 2022.

C/A: A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator.

PIC must provide pest control records to EHS via email within 10-days; PIC must also print or ensure records are maintained on the premise for the past 18 months to be reviewed by the department.

COS: 12/11/23- PIC provided all missing pest control records via email to EHS; records printed and added to file

Observed faucet fixture leaking water in restroom of room 116.

C/A: Repair/replace; Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair. Walls, floors, and ceilings shall be constructed of easily cleanable nonabsorbent materials and shall be kept clean and in good repair. While being used by tourists, every surface of a bathtub, shower, shower enclosure, toilet and lavatory, which may come in contact with a person’s body, must be sanitized each day, unless the guest has declined regular tourist room services. If a tourist declines regular tourist room services, the tourist accommodation facility must ensure that these surfaces are cleaned and sanitized at least once per week and between tourists
