Observed inoperable ventilation in the following rooms: 103, 105, 104. CA: ventilation must be in working condition in order to prevent odor, mold, mildew, and moisture

Observed numerous stains and burn holes on the bedding and pillowcases of the following rooms: 102, 104 and 108. CA: All burned and stained sheets/comforters must be thrown out/not placed in rooms. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean

Observed ventilation not working in the following rooms: 103, 104 and 108. CA: Toilet, lavatory and bathing facilities shall be mechanically ventilated. Ventilation must prevent odor, mold, mildew, and moisture

1. Observed a/c vents/filters soiled with accumulated dust inside a/c units in rooms 103, 105, and 226.
2. Observed heavily stained sofa chair in room 103 and sofa chair soiled with debris and dirt in room 226.

C/A: All furniture, draperies, appliances, carpets, and other accessories, and must be maintained in good repair, clean, and free of vermin

Observed lavatory faucet leaking water when turned on in room 226.

C/A: Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair

Observed hot water temperature exceeding 120F in rooms 120, 115, and 109.

C/A: Hot and cold running water under pressure shall be provided to all lavatories, bathing facilities, laundry facilities, and water-using equipment where eating and drinking utensils are washed. In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120F.

COS: All hot water heater thermostats adjusted to 119F during inspection

1. Observed baseboard unattached from lower wall in room 120.
2. Observed large hole in wall around sprinkler in room 120; observed hole around shower head in restroom of room 120.

C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed pink accumulation along interior chute of ice machine.

C/A: Ice machines shall be stored in a clean place and shall be kept clean

Observed accumulated dust along a/c filters and vents of air conditioning units in all rooms observed.

C/A: All furniture, draperies, appliances, carpets, and other accessories, in the tourist accommodation, whether the property of the tourist accommodation owner or not, shall be considered the property of and furnished by the tourist accommodation for purposes of enforcement of this Chapter, and must be maintained in good repair, clean, and free of vermin

1. Observed soiled/stained pillows/ pillow covers along beds in all rooms observed.
2. Observed soiled stained mattress in room 222.
3. Observed soiled/burned mattress cover along bed in room 103, 205, and 223.

C/A: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed toilet paper holders in disrepair, un-attaching from wall in restroom of rooms 222, 223, and 205.

C/A: Repair; Toilet rooms and fixtures must be smooth and nonabsorbent and shall be kept clean and in good repair

All rooms observed to have stains, holes, hair and/or burns in bedding. CA: Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be maintained clean

Observed room #213 had window with no screen. Both doors on floor 1 observed to be propped open as well. CA: Openings to the outside shall be effectively protected against the entrance of rodents and insects by tight-fitting doors, closed windows, screening, controlled air currents or other means

Observed the ice machine on the first floor with large brown and yellow build-up. CA: Ice machine shall be maintained in good repair and clean
