Regulation: 511-6-1.07(3)(a) - handwashing cleanser, availability (pf)
Observed no soap at the hand sink located next to the fryer. C/A Handwashing Cleanser, Availability. Each handwashing sink or group of two adjacent handwashing sinks shall be provided with a supply of hand cleaning liquid, powder, or bar soap. Pf COS Owner replaced the soap at the sink

Observed no paper towels at hand sink located in back kitchen area near dishwashing area. CA: Ensure all hand sinks are fully stocked with paper towels at all times. COS: Paper towels replenished.

Observed both hand sinks ditty with grease build up all over the sink. Observed the handwashing sink located the the rear of the facility with a fly swatter across it and a red funnel inside the sink. A handwashing sink shall be maintained so that it is accessible at all times for employee use. Pf
- A handwashing facility may not be used for purposes other than handwashing. Pf COS fly swatter and red funnel was removed. Owner was instructed to was the sinks C/A
15C - nonfood-contact surfaces clean
Regulation: 511-6-1.05(7)(a)2,3 - equipment, food/nonfood-contact surfaces, and utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (c)
Observed the paper towel dispenser located in the main kitchen next to the fryer caked in grease. Observed a box of hairnets located on the shelf in the main kitchen glued to the shelf and covered in grease. Observed the door handles to the coca cola fridge with heavy food build up. Container exterior food bin lids have food debris and residue build up. The fan and internal portable air conditioner unit has grease and fuzz build up. C/A The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations.
- Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
COS Owner has 3 days to corect the violation and submit pictures to EHS
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(3)(f) - lighting intensity, adequate in food prep, storage & service areas (c)
Observed the light in the WIC to be very din can barely see the food inside of it. C/A At least 10 foot candles (108 lux) at a distance of 30 inches (75 cm) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning;
- At least 20 foot candles (215 lux):
(i) At a surface where food is provided for consumer self-service such as buffets and salad bars or where fresh produce or packaged foods are sold or offered for consumption;
(ii) Inside equipment such as reach-in and under-counter refrigerators;
(iii) At a distance of 30 inches (75 cm) above the floor in areas used for handwashing, warewashing, and equipment and utensil storage, and in toilet rooms; and
- At least 50 foot candles (540 lux) at a surface where a food service employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor.
COS Owner has 3 days to correct the violation
17D - adequate ventilation and lighting; designated areas used
Regulation: 511-6-1.07(4)(b) - designated areas for employee activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (c)
Observed employee food mixed in with customer food in the Coca cola refrigerator, WIC and the prep contertop. C/ADesignated Areas for Employee Activity.

Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination.

Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur. COS Owner has 3 days to correct the violation and have a designated area for personal food
