1. Observed bathtub faucet leaking in room 405.
2. Observed slow draining sink in room bathroom of room 201.

Observed stains on pillows in rooms 515, and 402. Observed stains on sheets in rooms 303 and 402. CA: Washable mattress pads or covers shall be used on all mattresses. Beds, mattresses, springs, slats, mattress pads, mattress and bed coverings, pillow and pillow covers shall be clean and free from vermin. Each bed shall be provided, as a minimum, with two sheets and one pillow and pillowcase. After each occupancy and upon the request of the occupant, sheets and pillowcases shall be changed with fresh laundered linens. During occupancy, linens shall be changed at a minimum frequency of not less than weekly

Observed black mold-like substance in the ice machine on the 200s floor. CA: the use of existing self-service ice-storage bins may be continued, provided that the machines are maintained in good repair and capable of being properly cleaned and sanitized according to the manufacturer’s recommendations

Observed stains on mattresses in rooms 313, 402, and 512; observed stains on linens/pillows in rooms 500, 411, and 512. CA: Washable mattress pads or covers shall be used on all mattresses. Observed chemical bottles without labels on house keeping carts on floors 3 and 5. CA: Housekeeper carts shall be so arranged that clean replacement supplies, clean linens, and cleaned and sanitized multi-use equipment and utensils shall be protected from soiled items being removed from each room. Soiled linens and refuse shall be placed in appropriate containers on housekeepers’ carts. Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. Cleaners and sanitizer shall be used according to the manufacturer’s use direction and label

PIC could not provide the annual gas fired equipment inspection. CA: All gas-fired equipment shall be inspected at least annually by a qualified licensed contractor or local fire authority, if available. Points to be inspected are proper construction and installation, malfunctions and adjustments of controls and burners, faulty heat exchangers and vent obstructions. Any defects found on inspection must be corrected by a qualified heating contractor prior to use of the equipment. Upon request, the operator shall provide evidence of inspection and/or correction of any deficiency

Observed housekeeping cart on the 3rd floor with spray bottles (one with blue liquid; one with yellow liquid; one with pink liquid) stored on cart without a proper label or label of what the chemical is. CA: Any spray bottle used in cleaning a tourist room or stored on the housekeeping cart shall be labeled with its contents. COS: Employee labeled basic name of cleaner (glass, furniture, and washroom cleaner)

CA: Toilet, lavatory and bathing facilities shall be mechanically ventilated. Where ventilation ducts are used, ducts from toilet rooms shall not be connected into return ventilation ducts to any other room. In existing tourist accommodations permitted prior to the adoption of this rule, the current ventilation must prevent odor, mold, mildew, and moisture

Observed in rooms 526, 425, and 326 the water temperature exceeding 120°F. CA: In all new tourist accommodations, and where possible in existing tourist accommodations, hot water in all guest rooms shall not exceed 120°F. POC: Lower thermostat on boiler
