C/A: A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises.

Observed missing carpet in hallway of 1st floor.

C/A: Floors, walls, ceilings. windows, doors and all other appurtenances shall be of sound construction, properly maintained in good repair and shall be kept clean

Observed unwrapped single-use cups in rooms 302 and 316.

C/A: Single- service items placed in a guestroom or dwelling units shall be commercially pre-wrapped to protect against contamination

Observed pink slime buildup on 3rd floor ice machine chute.

C/A: Ice machines shall be maintained clean to sight and touch

Observed hot water temperatures exceeding 120 degrees Fahrenheit in rooms 312 and 421. C/A: In all guest rooms, hot water shall not exceed the 120-degree Fahrenheit mark

Observed stains on the pillow covers, pillows, and mattresses in rooms 131, 208, 406, and 421. Observed hair on towels in rooms 312 and in between sheets in room 406. C/A: Bed, bedding, and linens must be clean and free from stains

Observed no ventilation in bathrooms of rooms 131, 208, 406, and 421. C/A: Ventilation systems shall be functioning at a capacity to prevent odor, mold, mildew, and moisture

Observed build up on ice machines on 1st and 3rd floors. C/A: Ice machines must be cleaned and sanitized a frequency to prevent the accumulation of mold and other debris

A missing and cracked panel was observed underneath the refrigerator inside room 120. CA Furniture must be in good condition

Stains on the pillow cases, sheets, duvet covers and mattress pads were observed inside rooms 120, 200, 301 and 402. CA Bedding and linens must be clean and maintained

Mold was observed inside the shower in room 120. Re-caulk the perimeter of the shower wall inside room 402. CA Toilet facilities must be cleaned and maintained. The ceiling was observed peeling on the ceiling inside the shower in room 301. CA Toilet facilities must be maintained. A white film was observed on faucet of the sink inside the restroom on the main floor near the front desk. CA Toilet facilities must be maintained and cleaned

A stain was observed on the mattress inside room 423. CA Properly remove the stain or it is recommended to replace the mattress

A stain which appears to be mold was observed on the wall above the shower inside room 423. CA Toilet facilities must be properly maintained

A stain was observed in the toilet inside room 313. CA Toilet facilities must be properly maintained

Re-caulk the area around the hand sink inside the men's restroom inside the lobby area. CA Toilet facilities must be maintained and in good condition

Mold was observed inside the ice machine on the 4th and 3rd floor. CA Equipment must sanitized to prevent contamination

Broken dressers were observed inside rooms 120 and 200. The door to the closet was off track due to a broken latch. CA Furnishings must be in good condition

Stains were observed on the mattresses inside rooms 200 and 311. Stains were observed on the comforter inside room 200. CA Clean and or replace the comforter and mattress

A pink substance and rust was observed around the perimeter of the tub inside room 311. CA Showers must be cleaned and maintained. Recommend re-caulking the area

Rooms 120, 200 and 311 had a strong mildew smell. CA Recommend replacing the carpet to help with the odor

Mold was observed inside the ice machines on floors 120 and 311. CA Sanitize equipment to prevent contamination

A broken picture frame and several stains on the lamp shades inside room 407 was observed. CA Furniture must be in good condition and clean

Stains were observed on the mattress cover and mattress inside rooms 206 and 122. CA Bedding and linens must be clean

The toilet was backed up and would not properly flush inside room 407. Stains were observed on the wall inside the bathroom. CA Toilets must be maintained and in good repair. Physical facilities must be in good condition
