Observed food in boxes stored on the floor in the walk in cooler. CA: Food shall be protected from cross contamination by not being stored on the floor. PIC removed food in boxes from the floor and place them at least 6 inches off the floor in the walk in cooler.

Observed CFSM certificate not posted in a conspcious area of the facility. CA: The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. PIC will post CFSM certificate in a conspicuous area of the facility.
2-2A - management knowledge, responsibilities, reporting

Observed no employee health policy or no documentation to support an employee health policy in the facility. CA: The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A food employee or conditional employee shall report the information in a manner that allows the CFSM and person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms. PIC will be given a copy of an employee health policy on today (02/02/2026) for employees to read and sign.
2-2E - response procedures for vomiting & diarrheal events

Observed that the facility does not have established procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment. CA: A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter. PIC will receive a copy of a clean-up of vomiting and diarrheal events information today (2/2/26).
17C - physical facilities installed, maintained, and clean

Observed wet mop sitting in mop sink not in use. CA: After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies. PIC will hang wet mop up to dry.

Observed no recent inspection report posted in the facility. CA: The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. PIC will post recent inspection report in a conspicuous area of the facility.

Observed no employee handwashing sign in the restroom. CA:A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. PIC will post an employee handwashing sing in the restroom.

Observed bottom of reach in cooler in the kitchen area accumulated with chicken juice. CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. PIC cleaned bottom of reach in cooler in the kitchen area that was accumulated with chicken juice.

Observed no trash can with a cover in the restroom at the facility. CA: A toilet room used by females shall be provided with a covered receptacle for sanitary napkins. PIC will put a trash can with a cover in the restroom at the facility.
