Observed several sour cream containers being used to store Pico, guacamole, and jalapenos. CA: Single-service and single-use articles may not be reused. COS PIC removed food products from sour cream containers and placed them into food grade containers.

Observed grease build up on the vent hood in the main kitchen area. CA: Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris. PIC will have vent hood cleaned.

Observed chopped green peppers and chopped white onions uncovered in WIC. CA: Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. PIC will cover chopped green peppers and chopped white onions in WIC by today.

Observed black build up in ice bins in front area of kitchen. CA: Equipment food-contact surfaces and utensils shall be cleaned frequently such as ice bins. PIC will have employees to wipe down ice bins properly in front area of kitchen and drain, clean, and sanitize at the end of the shift.

Observed stained ceiling tile in dry storage area of facility. CA: Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. PIC will have stained ceiling tile replaced in dry storage area.

Observed raw chicken in the RIC near the grill area temping above 41 degrees Fahrenheit. CA: Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below. PIC tossed chicken that was out of temp.

Observed no Employee health policy. PIC stated that it was locked up in the office and the owners do not want the PIC to have access to the office. CA: The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A food employee or conditional employee shall report the information in a manner that allows the CFSM and person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms, if the food employee or conditional employee. PIC will be given a copy of the EHP for employees to sign and be kept available when the owners are not at the facility.

COS PIC had employees cover food at the steam table and cover taco shells on top of the oven.
