Observed no records of Employee Health Agreements or staff not able to explain or answer questions on Employee Health. CA: Keep agreements onsite and ensure all staff are educated on Employee Health.

observed hand sink with broken soap dispenser and not fully supplied and other hand sink with scrub brush and spoon stored inside. CA: Ensure all hand sinks are fully stocked and used for handwashing only.
