Observed build-up on white baffle in ice machine, inside unsweet tea urn nozzle, and inside multiple food storage bins. Equipment food-contact surfaces and utensils shall be clean to sight and touch.

Observed mold-like substance on several cabbages. Food disposed of during inspection.

Observed multiple flies throughout the kitchen area and rodent droppings on floor of dry storage area. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises.

Observed ice cubes on top of chicken, pork, and beef stored in prep top. Unpackaged food may not be stored in direct contact with undrained ice.

Observed fly light over food prep area. Insect control devices shall be installed so that the devices are not located over a food preparation area.

Observed heavy build up on shelves in dry storage area, walk-in cooler, bottom of reach-in cooler, and on equipment throughout the kitchen. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Observed build-up on walls, ceilings, and floors. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Observed employee restroom door propped open. Door closed during inspection.

Observed low floor grout throughout kitchen area, torn ceiling tiles, and broken floor tiles in walk-in freezer. All physical facilities shall be maintained in good repair.
