Observed no pest control records on file at the establishment. A record must be maintained on file and available at request of Health Authority for no less than 18 months of any pesticide use on the premises, except for the occasional use of consumer insect sprays on small spots in accordance with the product label. The record may be made by the permit holder or provided by the applicator, and shall list the following information: (a) amount and concentration of product used; (b) name of product used; (c) date and location of application; (d) application method; (e) pest targeted; and (f) name of applicator.
