Observed soiled encrusted food deposits on several pizza screens. Can not have deposits or encrusted deposits on food contact surfaces such as the pizza screen. PIC will have them cleaned by 03/19/25.

Observed soiled nonfood contact surfaces of rolling carts, shelves (rust), paper towel dispenser, soap dispenser, 3-compartment sink area, etc. PIC will have everything cleaned within two to three days.

Observed employee waiting on customer, touching register, touching payment from customer, and then open and handle/touch provolone cheese without washing her hands first. Employee did not wash after touching/handling payment. COS: Employee discarded contaminated food and then washed her hands properly.

Observed wet stacking of plastic food container in back dishwashing area on shelf. Observed several of the containers wet stacked. Discussed with PIC the proper air drying procedures.

Observed employee with beard/goatee longer than 1/2" handling/preparing food. Beards/goatees can not be longer than 1/2. Will wear beard nets or trim to 1/2" or less.

Observed hot water leaking at mop sink and observed damaged wall at mop sink due to a repair. The wall had to be cut out at mop sink. Will correct plumbing leak within three to five days and repair the wall within 30 days.

Observed cold holding TCS (Temperature Controlled for Safety) foods above 41°F in the Advantedge unit. TCS foods must be cold held at 41°F or below. See temperature log for said items. COS: PIC removed all items. Do not use unit until it can maintain proper temperature.
