Regulation: 511-6-2-.14(5) - condition of furnishings
Observed soiled shelving and refrigerator in room 134. PIC stated that shelving and refrigerator would be cleaned within 24 hours

Observed shower head plumbing cap damaged in room 140 leaving hole in shower wall. Ensure that all restroom fixtures remain clean and in good repair

Regulation: 511-6-2-.13(1) - insect and rodent control
Complaint investigation of Room 136 revealed evidence of rodent infestation. Observed large amount of rodent droppings behind dresser in left corner of room. Observed large amount of rodent droppings between box spring and mattress. Observed evidence of rodent droppings in Room 135 in corner of room near right bed and along wall behind beds/nightstand. Observed evidence of rodent droppings in Room 141 in corner, behind bathroom door, and in corner behind toilet. Owner was present at time of inspection of rooms and saw evidence noted.
Owner is to contact pest control company for treating rodent infestation by COB 5/21/25. Owner is to provide plan of corrective action developed by Pest Control professional for treatment and remediation of rodent infestation by 5/21/25, 5:00 PM.
Owner has been notified, in lieu of permit suspension, any rooms not rented at time of inspection are considered closed. Rooms cannot be reopened until ALL rooms have been cleaned to sight and touch, ALL furnishings cleaned, sanitized and disinfected, ALL bedding and linens have been washed and cleaned.
All rooms closed on 5/20/25 must remain closed until inspected and approved by EH to reopen. Discussed with owner it is recommended to clean blocks of rooms and then call for inspection of those rooms.
Owner is to contact EH by phone (2295154341) for reinspection. Upon request, EH will provide a reinspection within 1 business day after request is made.
Owner is aware that failure to comply with the above and noted timeframes may lead to permit suspension

Observed rodent dropping as noted above in Room 136. Observed rodent droppings on top of bedspread in Room 135. ALL linens, sheets, mattress pads, mattress covers and bedspreads in ALL motel rooms are to be removed from rooms and washed. Linens, sheets, mattresses, mattress covers and bedspreads will be inspected when rooms are requested to be inspected and reopeneded

Observed accumulation of dust and other debris in corners of bathrooms, behind bathroom doors, and under or behind fixtures. Owner stated that most rooms have not been rented in 1-2 months. ALL motel room's bathroom floors, walls, and ceilings are to be clean to sight and touch by 5/23/25. Bathrooms will be inspected in room by room basis as other rooms are inspected and allowed to reopen

Observed ALL rooms inspected having an accumulation of dust and debris on floors, in corners of rooms, behind or under beds, furnishings, curtains etc. Owner stated most rooms have not been rented in 1-2 months. Advised owner some rooms have not likely been cleaned, swept, or vacuumed since last visit. ALL motel rooms are to have floors, walls and ceiling cleaned to sight and tough by 5/23/25. Rooms will be checked and inspected on a room by room basis as they are inspected to be reopened

14 (5); Observed refrigerators in Rooms 151 and 153 having very rusty exteriors and interiors, and rusty shelving. Owner stated she was aware of the refrigerator conditions. Owner stated her husband had ordered 8 new refrigerators to replace in these rooms and other rooms. She stated refrigerators will be replaced as soon as they are delivered. Replace rusty refrigerators with new refrigerators by 12/31/24

14 (5); Dresser in room 153 was in poor condition. Dresser top was heavily stained and had excessive water damage on top of dresser. Damaged dresser is to be discarded. Owner stated dresser would be replaced with another dresser in better condition already in the room. She stated they have not had time to remove damaged dresser. She stated her husband will swap dressers out as soon as he can. Replace dresser no later than 12/31/24

Observed vanity sink in Room 121 to have/produce very little flow and pressure. Owner stated he was unaware of sink issue and that he will check faucet and repair or replace faucet as quickly as possible

Observed older refrigerators in Rooms 144 and 153 have excessive rust on outside of refrigerator. Also inside shelving shows evidence of excessive rust. Owner stated she will swap out refrigerators with new or better units within 30 days

Observed dust and debris build up on floors in Rooms 144 and 153. Observed cobwebs in upper corner along wall in Room 140. Owner stated she has had housekeeping cleaning issues lately. She stated she will go back and sweep and clean noted rooms to sight and touch tomorrow. Advised owner to check other rooms not inspected and clean as needed.
Repeat Violation: Observed dirty and dingy walls in rooms and bathrooms in all rooms inspected. Owner stated all rooms were scheduled to be painted in December. She stated the rooms were scheduled to be painted and she waited on contractor for four months and he never came to do job. All rooms are expected to be painted by next inspection
