13-201(k): At all times an adequate supply of toilet paper and liquid soap must be in all restrooms. Observed: boys restroom missing soap, girls restroom missing toilet paper.

13-190(a)- Equipment shall obtain turnover rate of 6 hours. Flow meter reads below minimum at 85 gpm. This is a repeating violation.
