Observed employee pull the phone out of their pocket while in the kitchen prep area. CA: Employees shall maintain a high degree of personal cleanliness and shall use good hygienic practices during all working periods in the food service establishment. COS: Discussed with PIC that phones are not allowed when working in the kitchen. PIC voluntarily put the phone away and washed hands.

Observed multiple time/temperature control for safety foods stored uncovered inside walk-in-freezer. CA: Food in packages, covered containers, or wrappings shall be covered except when cooling food. COS: Lids placed on all containers.

Observed uncooked ramen noodles stored uncovered on top shelf above prep-top-cooler: CA: Food in packages, covered containers, or wrappings shall be covered except when cooling food. COS: A lid was placed on container.

Observed seasonings in working containers not labeled with its common name near hand sink. CA: The label shall bear the common name to identify product.

Observed walls behind produce and 3 compartment sink are unclean to sight and touch.

Upon arrival, observed employee's cell phone stored on top shelf above prep-top-cooler. CA: Lockers or other suitable facilities shall be provided and used for the orderly storage of employees' clothing and other possessions. COS: Relocated cell phone.

Upon arrival, observed employee's personal unopened can beverage stored on prep table. CA: Areas designated for employees to eat, drink, and use tobacco shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. COS: Person-in-Charge voluntarily discarded beverage.
