6-1A - proper cold holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; cold holding (p)
Observed several food items not being held at 41 degrees and below. TCS food items include cantaloupe, honey dew melon, tomato salsa, and chopped lettuce. Food items were discarded.
6-1B - proper hot holding temperatures
Regulation: 511-6-1.04(6)(f) - time/temperature control for safety; hot holding (p)
Observed TCS foods not holding at 135F or more. TCS food items include pepper chicken, crawfish, cabbage, salmon, shrimp, sweet and sour chicken. Advised to remove double pans (several) from the steam wells.
6-1D - time as a public health control: procedures and records
Regulation: 511-6-1.04(6)(i) - time as a public health control (tphc) (p, pf, c)
Pic failed to provide time as a public health control procedures for food item. No records or discard times were observed for food on buffet line. Advised pic to keep records for food on TPHC. Provided TPHC document.
2-2A - management knowledge, responsibilities, reporting
Regulation: 511-6-1.03(2)(o) - person-in-charge duties (pf)
PIC was unable to to provide or demonstrate the knowledge or responsibility for reporting. Provided Pic with reporting agreement document.
2-2E - response procedures for vomiting & diarrheal events
Regulation: 511-6-1.03(6) - clean-up of vomiting and diarrheal events(pf)
Pic failed to provide clean up procedures for vomit and diarrhea events. EPA approved sanititizer not used. Observed non approved sanitizer bleach. Advised PIC that he must use epa approved sanitizer. Provided pic with chlorine bleach procedures.
4-2B - food-contact surfaces: cleaned & sanitized

Observed non epa approved sanitizer being used to clean food contact surfaces. Advised pic to only use epa approved sanitizer.
10D - food properly labeled; original container
Regulation: 511-6-1.04(4)(d) - food storage containers identified with common name of food (c)
Observed several dry bulk items (flour, sugar) not labeled that were out of their original container. Informed PIC to label all food items out of their original container. Containers were labeled.
15B - warewashing facilities: installed, maintained, used; test strips
Regulation: 511-6-1.05(2)(cc) - warewashing, automatic dispensing of detergents and sanitizers (pf)
Automatic dishwasher was observed dispensing chlorine sanitizer at 10 ppm. Chlorine should be dispensed at 50 ppm- 100 ppm. Until dishwasher is repaired, all dishes must be washed in the 3 compartment sink.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(2)(a) - floor, walls, & ceilings, cleanability (c)
Observed storage room with floors that contained debris, food particles, and dead roach parts. The areas needs to be cleaned better.
