Observed chicken being thawed in stagnant water upon entry. COS items were removed and placed in walk in cooler.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(j) - hair restraints (c)
Observed food employees without hair restraints upon entry. COS employees donned hair restraints. CA. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch.
12B - personal cleanliness
Regulation: 511-6-1.03(5)(f) - fingernails (pf)
Observed employee handling food with long painted nails. COS. employee washed hands and donned gloves. CA. Employees shall keep their fingernails clean and trimmed to no longer than the tips of the fingers. Pf Unless wearing gloves in good repair, a food employee may not wear fingernail polish or artificial fingernails when working with exposed food.

Most recent inspection report was not posted. C.A. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away. Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information.

Observed card board on shelf above fryer being used to absorb grease. COS cardboard was removed. CA.. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
15C - nonfood-contact surfaces clean

Observed build of food debris inside warmer cabinet in main kitchen. CA Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(a),(b) - good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)
Observed heavy build up of grease and debris on floors (beneath fryers and heat lamp), and build up of dust on the back of screens and ceiling tiles. Observed broken tile holding stagnant water at entrance door to walk in cooler and open area in front 3 compartment sink. Also observed missing open ceiling tiles in ware wash area. CA Floors, walls and ceilings shall be maintained in good repair and shall be kept clean to sight and touch. PIC advised to continue thorough cleaning of all areas of facility and replace missing floor and ceiling tiles.
