Staining was observed on bed linens, mattresses, pillows, and mattress pads in Rooms 134, 128, and 146.
All bedding components, including linens, mattresses, and mattress pads, must be maintained clean and free from stains Corrective Action: Remove, clean, or replace all affected bedding materials
16 - toilet/lavatory/bathing facility fixtures and rooms clean and maintained; properly lighted, clearly marked, proper signage, and all in good repair

In the rooms inspected, multiple violations were observed in the toilet and bathing facilities, including: bathroom doors and door frames in disrepair; stained and/or damaged ceilings in vanity areas; dirty bath tiles; holes in the walls and ceiling; and loose sink faucets.
All bathroom fixtures, finishes, and components must be maintained clean, in good repair, and properly secured
Corrective Action: Repair or replace damaged doors and frames, address stained or damaged ceiling surfaces, thoroughly clean bath tiles, and repair or replace all loose or broken hardware, including sink faucets
17 - proper ventilation; no sign of mold, mildew, odors or moisture
Bathroom exhaust vents in rooms inspected were observed unclean to sight and touch, with visible dust and debris accumulation. All ventilation components must be maintained clean and in good condition to prevent buildup and ensure proper air quality. Corrective Action: Remove and thoroughly clean all bathroom vent covers and internal components to eliminate dust and debris. This was discussed with the person in charge
25 - floors, walls, ceilings, windows, doors clean, and in good repair; appropriate for location and operation
Regulation: 511-6-2-.14(1) - floors, walls, and ceilings
Walls and ceilings in Rooms 128, 134, and 146 were observed heavily stained and unclean to sight. Bathroom vanity doors were in disrepair, with damage and missing knobs. Outlet covers were also in disrepair, leaving openings in the walls exposed. In Room 128, buildup was observed on walls in the kitchenette area near the stove. Baseboards in the rooms inspected were observed chipped, stained, and/or damaged.
All surfaces, fixtures, and structural components must be maintained clean, in good repair, and free of damage
Corrective Action: Clean and restore all stained walls and ceilings. Repair or replace damaged vanity doors and install missing knobs. Replace or repair outlet covers to properly seal wall openings. Thoroughly clean buildup on kitchenette walls, particularly near cooking areas. Repair, refinish, or replace damaged baseboards and clean all surfaces to remove stains and buildup
32 - separate, structurally sound, clean, and in good repair: dryers vented and adequate lighting
The guest laundry room was observed unclean to sight, with accumulation of debris and buildup present. Areas behind the washer and dryer require cleaning. The laundry exhaust fan was also observed with excessive dust and soil accumulation.
All laundry facilities must be maintained clean and free of debris, including hard-to-reach areas and ventilation components.
Corrective Action: Thoroughly clean the guest laundry room, including behind and around all equipment. Clean the laundry exhaust fan to remove accumulated dust and buildup
