Observed food containers of ham, pineapples, jalapeños, lettuce, cheese, broccoli, and banana peppers stored under the prep top cooler without any lids or coverings. CA: Food shall be stored in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination. COS: PIC provided lids for each food container under the prep top cooler.

CA: Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. Observed food employee wearing bracelets in the main kitchen while prepping food. COS: PIC had food employee remove the bracelets.

Observed Pizza on rack using time as public health control without proper labeling. COS: PIC added discard time

Facility was also unable to provide written procedures. Provided black TPHC form.
