4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(7)(a)1 - equipment, food-contact surfaces,& utensils (pf)
Observed chicken residue, flour, left on multiple dishes stacked as clean. PIC stated that the overnight crew does not do a good enough job and they have to redo them during the morning shift. CA: Equipment food-contact surfaces and utensils shall be clean to sight and touch.
4-2B - food-contact surfaces: cleaned & sanitized
Regulation: 511-6-1.05(6)(n) - manual and mechanical warewashing equipment, chemical sanitization-temperature, ph, concentration, hardness (p,pf)
Observed Quaternary ammonium sanitizer at sink. CA: A quaternary ammonium compound solution shall have a minimum temperature of 75ºF (24ºC), P have a concentration as specified in DPH Rule 511-6-1-.07(6)(g) and as indicated by the manufacturer’s use directions included in the labeling, P and be used only in water with 500 ppm hardness or less or in water having a hardness no greater than specified by the EPA-registered label use instructions. COS: Quaternary solution @ 3-compartment sink 200ppm after corrections.
15B - warewashing facilities: installed, maintained, used; test strips
Regulation: 511-6-1.05(3)(h),(i),(j) - temperature measuring device, manual warewashing; sanitizing solutions, testing device (pf)
Lack of use of chemical test kit when using chemical sanitizer at three-compartment sink/warewashing machine. CA: Concentration of the sanitizing solution shall be accurately determined by using a test kit or other device. COS: testing strips were provided near the 3-compartment sink. PIC has been advised to put strips in area with employee who has task of changing solution.

Observed soiled reach-in cooler shelves. Observed grease accumulated on kitchen floor. Observed attached equipment soiled with accumulated food debris. Observed food debris accumulated on kitchen floor. Observed wall soiled with accumulated grease.
CA: Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
17C - physical facilities installed, maintained, and clean
Regulation: 511-6-1.07(5)(f) - drying mops (c)
Observed mop being stored in unclean water.
CA: After use, mops shall be placed in a position that allows them to air-dry without soiling walls, equipment, or supplies.
