Facility does not have CFSM certificate posted for public view.CA: The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority.
14B - utensils, equipment and linens: properly stored, dried, handled

Observed forks at the consumer self service area were not pre-wrapped and the food/lip-contact surfaces were presented to the consumer (the handles were not presented to the consumer.)
COS: Person in charge voluntarily discarded and replaced forks.
CA: 2. Knives, forks, and spoons that are not pre-wrapped shall be presented so that only the handles are touched by employees and by consumers if consumer self-service is provided.
15B - warewashing facilities: installed, maintained, used; test strips
Regulation: 511-6-1.05(3)(h),(i),(j) - temperature measuring device, manual warewashing; sanitizing solutions, testing device (pf)
Facility lacks test strips for their sanitizing solution(quats). CA: A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided.
