Observed dish machine ready below manufacturer's levels of recommendation at 0ppm. COS: After several runs, maintenance primed dish machine and now reads 100ppm cl. CA: Chemical Sanitization - Temperature, pH, Concentration, and Hardness. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instruction.

Chlorine sanitizer measured below 50ppm at mechanical warewashing machine. Advised person-in-charge chlorine sanitizer shall measure between 50-200ppm. COS: Person-in-charge removed food-contact surfaces from dish machine to wash, rinse, sanitize at 2 compartment sinks.

Observed 3 gallons of milk in the reach in cooler expired on 02/06/2025. CA: Discussed with the person in charge that prepackaged sandwiches, eggs, infant formula, shucked oysters, milk, and time/temperature control safety foods that are labeled as “keep refrigerated” and that are for sale or service to the consumer or used as an ingredient in other foods shall be immediately discarded and shall not be sold, served, or used after the manufacturer’s expiration date or the sell-by date. COS: Person in charge discarded the milk.

Observed bowls are stored on the parchment paper. Discussed with the person in charge that utensils and/or food contact surfaces of equipment are not made of materials that are safe, durable, corrosion-resistant, and nonabsorbent.

Observed black residue on white liner inside ice machine. CA: Food contact surfaces shall be kept free of accumulation. Advised person-in-charge to drain ice, wash, rinse, and sanitize ice machine. Then make ice, drain the ice, finally fill ice machine with ice.

Facility lacking testing device to measure chlorine and quat ammonium sanitizer. COS: Provided chlorine and quat ammonium sanitizer strips.
