The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority. A CFSM certificate which has expired, been revoked or suspended shall not be posted in the food service establishment. Observed CFSM certificate not posted in public view in each food service establishment. PIC was advised to have CFSM certificate posted in public view by 2/23/26 and email picture to inspector.

Post the permit in a location in the food service establishment that is conspicuous to consumers. Permit was deemed invalid and not posted due to unpaid permit fees. NOV was issued due to non-payment of permit fees. PIC was advised to pay permit fees no later than 2/19/2025.Failure to voluntarily correct the problem by that date may result in legal action.
